Senior Network Coordinator

  • Responsible for information dissemination. The Senior Network Coordinator will be the GLPTN's point of contact and will communicate program related information to the Network Director, Program Manager and Board of Directors, as well as other network personnel, quality improvement advisors, and external stakeholders. The Network Coordinator will monitor the GLPTN's development and provide leadership support to state operations teams ensuring effective and timely implementation of planned activities in GLPTN provider areas, and will monitor the reporting of these teams to ensure timely data feedback loops are established. 

  • Responsible for tracking provider commitments, metrics for CMS reporting and connecting local entities with the national CMS resources available to assist in practice transformation.

  • Assist in the assessment of network needs, by performing ongoing environmental scans, and in the development and implementation of strategies to improve the performance of operation teams and may also represent the GLPTN at meetings and workshops.

  • Develops and implements policies, procedures and guidelines for network implementation per CMS requirements.

  • Coordinate all aspects of Quality Improvement Advisor training activities and GLPTN events, including location, technical assistance, content delivery, etc.
